What to Bring to Your Appointment

What you need to know before visiting OIC

We want to make your child’s and family’s visit to OIC an easy one. We’ve provided helpful information about preparing for your child’s appointment, including what to bring, questions to ask, and directions to OIC.

Upon arrival, our staff will review your information and provide you with any forms still needed. They’ll then inform the medical staff that you and your child have arrived.

We pride ourselves on making sure your child is seen in a timely manner. Unfortunately, sometimes emergencies and complications occur that may lead to unexpected delays.

Help your child’s appointment run smoothly by bringing the following items with you: 

  • Your child’s insurance card
  • Your picture ID
  • Court documents showing you have legal guardianship (if you are not the child’s parent)
  • List of the child’s current medications
  • If your child is new to OIC, please also bring the child’s medical records (if available)

If your child has already been seen in an emergency room (ER) for the injury, please also bring:

  • Any X-rays taken in the ER (the ER should be able to provide these to you on CD)
  • Discharge papers (when available)
  • Any X-rays taken by past healthcare providers (these can usually be given to you on a CD)
